What is a Community Health Champion???

A Community Health Champion is someone who...

  • Has worked to improve the health of family, friends, and neighbors in their professional capacity, as a volunteer or on their own! He or she does not need to be a health care provider or work in the medical field, although we certainly like to honor providers who go above and beyond the call of duty. A community health champion could be a coach, club or organization member or just someone who goes out of his or her way to help people lead healthier, happier lives.
  • Is one of the unsung heroes making a difference in others' lives.
  • The Health Initiative wants to recognize and thank for their service to the community!

Community Health Champions are recognized with a certificate and are featured on the website, in local newspapers and at our annual meeting in October.

Nominations are accepted on a rolling basis. Awards are given out quarterly.
